Insurance Payment

Out of Network Benefits

If your insurance includes Out of Network benefits:

  • Pay the full cost of each session until your reach your Out of Network Deductible amount
  • After you reach your deductible, you will pay a portion of the cost and your provider will pay the rest (Contact your insurance provider about this)

If you do not have Out of Network benefits:

  • You would pay out of pocket for each service, see fees below

Out of Pocket/Private Pay

If you do not have insurance

  • You are Out of Network if we do not accept your insurance. If your insurance does not have Out of Network Benefits, you can choose to pay privately or use your Health Spending Account (HSA) for your services here at Bethany Health Services. (Contact your insurance provider about this)

Payment

Co-payments, co-insurance, and out-of-pocket fees are due at the beginning of the service.

PAYMENTS ACCEPTED: CASH, CREDIT, HSA, FSA

  • You may also use your client-patient portal pay using a credit card. Please feel free to email us at: info@bethanyhs.com

Fees

If you do not want to use your insurance to cover your appointment due to privacy reasons or out of network.

Psychiatric Intakes/ Medication Appointments

  • Initial Intake Psychiatric Assessment-45-60 minutes- $350
  • Initial Intake Assessment-greater than 60 minutes $400
  • 15 Minutes follow up – $150
  • 20-30 Minutes follow up -175
  • 45 Minutes follow up – $200
  • 60 Minutes follow up- $250

Bundle Discounts: We offer discounts if services are paid in full for 3, 6 or 12 month increments:
5% off for 3 months, 7.5% off for 6 months and 10% off for 12 months

Hormone Replacement Therapy (Include Consultations, labs and medications)

  • Initial Hormone Replacement – $500
  • Monthly Recurring Fees – $225
  • Bundle Discounts: We offer discounts if services are paid in full for 3, 6 or 12 month increments:
    5% off for 3 months, 7.5% off for 6 months and 10% off for 12 months

Medical Weight Management (Includes consultation, medications and labs)

  • Monthly fees
    • Semaglutide- $500
    • Tirzepatide – $800

5% off for 3 months, 7.5% off for 6 months and 10% off for 12 months
